I currently collect and process the following information:

• Information related to your your past and medical history, including test results or reports you share.

• Details about your lifestyle and personal situation.

How I get the personal information and why I have it

Most of the personal information I process is provided to us directly by you for one of the following reasons:

• I need to collect personal information about your health in order to provide you with the best possible treatment by planning and tracking your treatment.

• I have a legitimate interest in collecting that information, to ensure I can practice acupuncture effectively and safely.

• Records are kept of your contact information so I contact you in order in cases as to update you on matters related to your treatment or remind- you of appointment times.

 

Your information will not be shared with third parties unless I have a legal obligation. Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing this information are:

(a) Your consent. You are able to remove this at any time. You can do this by contacting info@acupuncturewithangela.com

How I store your personal information

Your information is securely stored on a secured onedrive on a machine that is password protected.

I have a legal obligation to retain your records for 8 years after your most recent appointment (or after you have reached age 25, if this is longer), after which time the records are deleted. Any paper notes will be uploaded online and shredded immediately.
